  1. What's your diagnosis by morphology?

    A: benign endometriosis

    B: carcinoma, favor GI origin

    C: endometrioid carcinoma

    D: High grade serous carcinoma

Answer

The correct answer is B. Specimen contains scattered clusters of atypical glandular cell in a background of necroinflammatory debris, consistent with adenocarcinoma. By imaging: endoscopic finding of a poorly defined 36 mm x 22 mm mass in porta hepatis in gallbladder fossa with several enlarged adjacent lymph nodes is noted. The most likely diagnosis is pancreaticobiliary origin.
