A: Stent effect and reactive atypia

B: Reactive atypia due to inflammation

C: Adenocarcinoma

D: Atypia, not carcinoma yet


Answer

The correct answer is C. This is a case of moderately differentiated adenocarcinoma likely cholangiosarcoma. It has malignant cytology: pleomorphism, prominent nucleoli, and irregular nucleus membrane.
